Job Description:

Under the direction of the Medical Coding Manager, the Physician Coding Auditing & Documentation Specialist reviews and audits medical records of outpatient clinic visits to facilitate accurate and complete medical record documentation to reflect clinical treatment and accurate capture of diagnoses in compliance with federal and state regulations and according to established ICD-10-CM coding and/or CPT-4 procedure coding classification systems.

This position identifies additional documentation opportunities relating to HCCs (Hierarchical Condition Categories) and provides appropriate education to YVFWC clinicians and other organization team members for documentation improvement in reference to ICD-10-CM and CPT-4 coding and documentation guidelines and requirements. Educational sessions may be provided via face-to-face in both small or large groups or electronically.

Schedule: Monday - Friday 8:00 a.m. - 5:00 p.m.

Essential Functions/Responsibilities/Duties
•Uses a thorough understanding of coding guidelines and standards of documentation compliance to improve overall quality and completeness of clinical documentation within the patient electronic health record (EHR).
•Conducts retrospective medical record audits of outpatient clinic encounters according to federal and state regulations and established ICD-10-CM and/or CPT-4 procedure coding classification systems.
•Identifies errors and issues related to coding and medical record documentation.
•Provides consultation and consistent, ongoing education and training to clinicians and other clinical staff regarding accurate coding and documentation standards.
•Researches, analyzes, recommends, and facilitates plan of action to correct discrepancies to prevent future coding errors.
•Identifies discrepancies, potential quality of care and billing issues.

Qualifications
•Education: High School diploma or General Education Development diploma (GED). Experience or coursework in medical terminology, health records, Health Insurance Portability and Accountability Act (HIPAA) and compliance required.
•Experience: Six months experience or coursework in medical terminology, health records, Health Insurance Portability and Accountability Act (HIPAA) required. Total of five years' experience in healthcare coding preferred. Three years' experience as a healthcare coding auditor preferred.
•Professional Licenses/Certificates/Registration: CPC (Certified Professional Coder) or Certified Coding Specialist (CCS) certification preferred. CDEO (Certified Documentation Expert Outpatient) certification or CCDS-O certification must be obtained within 24 months of hire.
•Knowledge/Skills/Abilities Required or Preferred: Knowledge of medical terminology, medical records, investigations, and auditing. Knowledge of healthcare coding, HIPAA, and State applicable privacy laws. Good analytical, decision-making, and problem-solving skills. Skillfully exercises independent judgment and decision making. Handles sensitive situations and confidential information with discretion. Effectively prioritizes work and handles a variety of tasks simultaneously environment. Work in a collaborative team environment, be well organized and have a strong attention to detail and accuracy. Ability to effectively present educational material to individuals, small and potentially large audiences. Strong interpersonal communication (verbal, non-verbal and listening) skills, including handling stressful situations. Develops effective working relationships to gain trust and establish credibility. Basic proficiency with a variety of computer programs including Microsoft Outlook, Word, Excel and Electronic Medical Record (EMR), preferably Epic.
